Summary of Benefits

All employees are eligible for the following benefits on the 1st of the month following 30 days of full-time regular employment, while working a minimum of 30 hours/week unless otherwise noted below. Benefits are subject to change at the discretion of Prescott Metal/EAM.

Medical Insurance

Prescott Metal/EAM offers a $2,500 deductible self-insured medical plan, administered by HPI (Harvard Pilgrim). We also offer a $5,000 deductible plan (Prescott pays the employee contribution portion of the premiums for the $5,000 deductible plan), and a High Deductible Health Plan $2,000 deductible with an HSA plan.

Dental Insurance

Dental insurance is administered by Principal Dental. Preventive covered at 100%, Basic services 80%, Major services 50% In-Network and Non-Network providers.

 Vision Insurance

Voluntary employee-paid benefit consists of $25.00 co-pay for lenses, $150.00 allowance for frames, elective contacts are $60 co-pay for standard and premium elective contacts (fitting & evaluation). Certain lens enhancements at additional cost. Non-network provider reimbursements. VSP Choice Network.

Flexible Spending Account (FSA)

The flexible spending account (FSA) allows employees to set aside pre-tax dollars to use for out-of-pocket medical/dental/vision and/or dependent care expenses. This benefit is entirely funded by the employee and the administrator is HPI.  The plan year for the FSA is 1/1–12/31.

Health Savings Account (HSA)
Health Equity administers the health savings account. The company matches employee contributions, employee only coverage $500.00 match, employee plus one or more dependents $1,000 match.

Group Life Insurance (including AD&D) Short-Term Disability and Voluntary Long-Term Disability

The group basic life and AD&D and short-term disability benefit is full paid by Prescott Metal/EAM and administered through Principal.  Eligible employees receive $10,000 in basic life and AD&D, at no cost to the employee.  Short term disability pays 66 2/3% up to a maximum of $1,000 weekly and is subject to an eight (8) day elimination period, at no cost to the employee.

Voluntary Life Insurance Guarantee issue to $100,00 of coverage without having to complete a medical questionnaire. Eligible employees can get up to $300,000 of coverage with acceptable medical history. Spouse coverage minimum of $5,000 to a guarantee issue of $30,000 and a maximum of $100,000 not to exceed 50% of your coverage. Child(ren) coverage is $10,000.

Legal Shield, Small Business Supplement (effective 11/16/2020) Membership includes: direct access to a law firm, legal advice, letters and calls on your behalf, contracts & document review, residential loan assistance, will preparation, speeding ticket assistance, IRS audit assistance, trial defense, divorce-separation-adoption-name change assistance, 25% discount on services for DUI, bankruptcy, criminal charges, personal injury, 24/7 emergency access.

Weekly deductions:

Legal Shield Individual: $4.37 per week

Home Business Supplement: $additional $2.30 per week

Combined: $6.67 per week.      

We also offer voluntary supplemental insurance plans which include accident and critical illness.

401(k) Retirement

Employees are eligible to begin contributing to the 401(k) plan if they are at least age 21 as of the first of the month following 30 days of service. Prescott Metal/EAM will begin a discretionary match of 50% of employee deferrals, up to a maximum of 3% after one year of service.  Employee deferrals will always be 100% vested.  The employer vesting schedule is 0% for less than 2 years of service and 100% after 3 years of service.

Vacation is based on an annual accrual.

Paid Vacation time  is available to eligible employees to provide opportunities for rest, relaxation, and personal pursuits, and it accrues annually based on the schedule below:

40 hours of Maine Earned Paid Leave (EPL) to be used as employees see fit.

 Education Assistance

After six (6) months of employment, employees may apply for education assistance up to a maximum of $1,000 per calendar year.

Tools & Equipment Assistance

New employees can use company-owned tools to start, and after three (3) months of employment, regular full-time employees receive a 50% discount off the purchase price of approved hand tools used for work.

Work Boot Assistance

Regular full-time employees are reimbursed a maximum of $100 per year for approved work boots that meet safety guidelines.

Safety Glasses

Prescott Metal/EAM will pay up to $100 for prescription safety glasses through a preferred provider.

On-Site Fitness Center
Includes free weights, pull-down machine, elliptical, treadmill, stationary bike, dumbbell racks.
